The Wellness Coalition is a non-profit organization with services that focus on improving access to healthcare for adults with chronic diseases who have limited or no health insurance in the River Region counties of Alabama (Autauga, Elmore, Lowndes, Macon, and Montgomery).

Case managers, also called Wellness Navigators, are available to assist clients with chronic conditions such as asthma, diabetes, heart disease, hypertension, obesity, mental illness, or any other chronic health condition.

Many services are offered to clients such as medication assistance, help with finding a medical home, and wellness goal setting, as well as various classes and programs, chronic illnesses.

Chronic Disease Self-Management Program

Our Chronic Disease Self Management Program (CDSMP) is a six-week program open to anyone who wishes to learn how to better manage their chronic condition or that of a friend or family member.

Heartlink

HeartLink is a program that seeks to improve cardiovascular health and health behaviors through:

  • Access to primary care
  • Health insurance enrollment assistance
  • Free/low-cost medications via community resources
  • Self-management of cardiovascular disease

Services within this program are delivered by Wellness Navigators who provide wellness case management and chronic disease self-management education.

Diabetes Prevention Program

Our Diabetes Prevention Program (DPP) is a one-year program that focuses on helping clients prevent or delay type 2 diabetes.

To qualify for this program, clients must be 18 or older, have a body mass index of 25 or greater, and have received a diagnosis of prediabetes from their physician or receive a score of 5 or greater from a prediabetes risk assessment.

Participants in the program learn new lifestyle habits to help them lose 5-7% of their initial body weight and incorporate 150 minutes of physical activity each week. See if you are at risk of prediabetes by taking a short quiz at bit.ly/CYL2TWC.

Wellness Case Management

Wellness Case Management is a service that can help you find a doctor and link you with medications and other services you need to manage a chronic disease. Your Wellness Navigators can also help you set wellness goals and help you work to reach them.

Wellness Navigators are also available to assist clients with applying for health insurance coverage through the Federally-facilitated marketplace,
also known as the Affordable Care Act or Obamacare, during open enrollment or special enrollment periods under certain circumstances.
