Get Support to Manage Type 2 Diabetes

The Wellness Coalition Diabetes Self-Management Program (DSMP) provides personalized services to help you manage type 2 diabetes. Diabetes is complicated, and it takes practice to manage it in your everyday life. You may be wondering about eating balanced meals, exercising safely, checking blood sugar, or giving yourself injectable medicines.

DSMP can help you improve your blood sugar levels to:

  • Prevent or delay complications

  • Avoid emergency care

  • Save money on healthcare costs

DSMP can also help you:

  • Lower your body weight

  • Decrease your blood pressure

  • Improve your cholesterol

How does DSMP Help Me?

DSMP helps you manage diabetes so you can prevent or delay serious complications like heart disease, kidney disease, amputations, and vision loss. People who participate in DSMP are more likely to have better overall health and improved quality of life with diabetes.

You’ll work with a diabetes care and education specialist who will help you create a plan and teach you about:

  • Healthy eating and being active
  • Taking medicine as prescribed
  • Monitoring blood sugar levels
  • Reducing risks to lower the chances of diabetes complications
  • Coping and emotional well-being
  • Finding solutions and take action

Is there a cost?

There is no cost to you. DSMP is covered by many insurance companies, including Medicare.

What will I get?

Through DSMP, you will:

  • Learn to manage diabetes using new knowledge, skills, and tools
  • Build confidence and emotional strength
  • Practice how to fit diabetes care into all of your daily routines
  • Get support (in person or online)

When should I sign up?

These are the best times to get started with DSMP:

  • If you’ve been recently diagnosed at doctors’ appointments

DSMP is Proven to Work

Every year, nearly 1 million people with diabetes receive DSMP services recognized or accredited by the American Diabetes Association or the Association of Diabetes Care & Education Specialists.

DSMP has a positive impact on lifestyle changes, such as eating patterns and activity levels. It’s been shown to decrease A1C levels, prevent or delay complications, and improve quality of life. On average, a person with diabetes spends less than 1% of their life with their healthcare team, so the focus of DSMP is to help people with diabetes develop problem-solving skills to self-manage diabetes.
