Position Title: RN Consultant – Cardiovascular Program
Reports To: HeartLink Program Coordinator
This is a contract consulting position for approximately 5 hours per month.
The Wellness Coalition’s HeartLink program seeks to improve the cardiovascular health and health behaviors of uninsured and underinsured adults in Alabama’s River Region, through:
- Access to primary care
- Health insurance
- Free/low-cost medications
- Community resources
- Self-management of cardiovascular disease, delivered by trained Wellness Navigators providing wellness case management and chronic disease self-management education
The HeartLink program is available to adult residents of Autauga, Elmore, Lowndes, Macon, and Montgomery counties.
Essential Duties and Responsibilities:
- Work with the HeartLink team to design an education program for people with cardiovascular disease or at risk for developing cardiovascular disease.
- Assist in development of cardiovascular-related wellness goals for use by navigators in individual education and goal-setting.
- Provide cardiovascular training for navigators and other staff.
- Advise Executive Director and other management staff regarding appropriate strategies and research related to cardiovascular health.
- Assist with planning & delivery of cardiovascular health-focused summit (or virtual webinars) for health workers including presentations as needed.
- Coordinate with program evaluators in developing instruments to evaluate the cardiovascular education program.
- Provide input into development of communications related to cardiovascular health.
- Assist with collecting participant data as required to meet program standards.
- Provide other services as requested or authorized by The Wellness Coalition Executive Director or designee.
